The aim of this activity is to increase patient safety and assist general practitioners in implementing and improving systems to incorporate advance care planning as part of routine healthcare, including raising the topic with all older patients. In addition, we will introduce the Care Plan for the Dying Person - Victoria (CPDP-Vic) clinical resource and describe how it can provide an effective framework to guide care decision making with imminently dying people, their families and the health professionals providing care in the hospital setting. There will also be a discussion about the recent changes to medical decision-making laws in Victoria.

- Explain the contemporary scope of palliative care and its importance in supporting the clinical management of older Australians
- List recent changes to medical decision-making laws and describe the impact on primary care
- Discuss the initiatives led by the Palliative Care Clinical Network (PCCN)
- List factors required for successful implementation of the CPDP-Vic
