GP Management Plans and Team Care Arrangements review: Prioritising patient’s end-of-life care needs

GPs, nurses, practice managers and other health professionals are invited to this webinar on GP Management Plans (GPMP) and Team Care Arrangements (TCA) and how they enable GPs to plan and coordinate the health care of patients living with chronic or life limiting illnesses.
Learning outcomes:
- Describe the role of GPs and multidisciplinary teams in structurally identifying palliative care needs of patients and their carers
- Explain how GPMPs and TCAs contribute to improving palliative care coordination
Presenter: Dr Shannon Welsh, General Practitioner - Dr Welsh is a Rural Generalist GP with a special interest in palliative care. Shannon spends most of his time working as a GP in Yarrawonga and has a weekly session with Moira Community Palliative Care. Prior to studying medicine, Shannon flew fixed wing and rotary wing aircraft as a pilot in the Royal Australian Navy. He went on to study medicine at Deakin University, then completed his internship with the Murray to Mountains program. Shannon has since held positions in emergency medicine, obstetrics, anaesthetics, palliative care and general practice. Shannon’s main clinical interests include skin cancer surgery, emergency medicine and palliative care.
