General Practice Investment Strategy

Murray PHN is making $1 million of funding available through its General Practice Investment Strategy to support people living with chronic disease in our communities.

Local general practices, including medical services in Aboriginal Community Controlled Health Organisations, were able to apply for up to $100,000 to implement or embed new models of chronic disease and/or complex care coordination.

The funding will be providing up to 20 practices with the opportunity to enhance the continuity of care and multidisciplinary team-based care that they provide and improve patient self-management and practice efficiency.

Key dates (2024)

  • Friday 1 March - request for proposals open
  • Wednesday 13 March - pre-qualification closes
  • Friday 15 March – questions close
  • Friday 22 March – proposals close
  • Thursday 3 to Friday 4 - interviews if required
  • Wednesday 10 April – respondents notified
  • Monday 29 April - agreements commence
  • Monday 1 July - activities start
