Murray HealthPathways | Progress update and survey
Murray PHN is  building HealthPathways, working with local health services and  clinical teams to design the best ways to provide timely and appropriate care.

The Murray HealthPathways website will be available in late 2016, to be used by clinicians at the point of care to access evidence based, locally accessible patient pathways.

To assist Murray PHN in preparing for the launch of Murray Health Pathways, please complete a short survey. Click here to participate.

Refugee immunisation | Resources

The Victorian Refugee Health Network Immunisation Working Group  has developed new resources to support access to catch-up immunisation for patients from refugee and asylum seeker backgrounds.  

Supercare pharmacies

The Victorian Government has committed $28.7 million to introduce 20 Supercare Pharmacies.  Pharmacies are strongly advised to contact Department of Health and Human Services if they are interested in being part of this initiative:

The  Chief Health Officer Advisory has released an alert about the changing pattern of Mycobacterium ulcerans infection (also known as Buruli ulcer or Bairnsdale ulcer)  in Victoria.

Integrated Team Care | Information sessions

Murray PHN invites expressions of interest from health service providers for the commissioning of Integrated Team Care (ITC) teams, commencing from 1 January 2017.  ITC teams work across mainstream and Aboriginal health services to improve access and coordination of care for Aboriginal and Torres Strait Islander people with chronic disease.

Located across the Murray PHN catchment area, each ITC team will include an  Indigenous Health Project Worker:  who supports improved collaboration and integration between health services, a  Care Coordinator: a  qualified nurse or Aboriginal Health Worker and an  Outreach Worker:  providing practical assistance to help patients access services.

Murray PHN is hosting four information sessions next week and you are invited to hear about Murray PHN’s approach to commissioning these services.

VicHealth awards

The 2016 VicHealth Awards will celebrate great health promotion projects that improve and maintain the health and wellbeing of Victorians. State and local governments, health agencies, non-government, not-for-profit and private sector organisations can apply.  Nominations close Friday 19 August.

New Urologist | Mildura

Dr Jos Jayarajan  MBBS(Hons), B Med Sci, FRACS now has a VMO appointment at Mildura Base Hospital, operating sessions at Mildura Private  and joins Mr Peter Liodakis consulting in Mildura as part of the North Eastern Urology team in September 2016.
